Office Information: About Your Visit

Appointments

Regular office visits are scheduled by appointment.
Our office staff will need to know:
* Your name and phone number
* The nature of your problem
* The name of your insurance company
* The type of insurance you have (PPO,POS,HMO, etc.)

Should you need to cancel or reschedule your appointment, we would appreciate being notified as soon as possible.

Dr. Fowler and his entire staff work to keep patient wait times to a minimum. Occasionally a patient arrives with special needs that alter our schedule. We appreciate your understanding of these situations.

Emergencies

Should you require orthopaedic services at times other than regular office hours, please call: 404-609-0410
Either Dr. Fowler or an orthopaedic surgeon in Dr. Fowler's call group will return your call.

Prescriptions

Medical staff authorizes new prescriptions and refills during regular office hours. When requesting a new script or a refill by phone, please have the name and phone number of your pharmacy available.

Financial Policies

Dr. David Fowler is a Provider for several insurance companies, including Medicare. He is not a Medicaid/PeachCare doctor. We will be happy to answer any questions you may have concerning types of insurance accepted in our office. All co-payments and deductibles are due at time of service, and the balance will be filed with your insurance company. If the physician is not a Provider for your insurance company, full payment is due at time of service. Your claim will be filed for you; any payment made will be refunded to you promptly.

Workers Compensation from injuries in the state of Georgia only is accepted with written verification to include claim number, date of injury, insurance name, address, phone number and the name of the adjuster.

Injury due to an automobile accident can be filed with your automobile insurance company with proof of medical coverage, and amount of coverage. If this information is not provided, full payment is due at time of service.

If you are not covered by any insurance, full payment is due at time of service.
